Software Purchase Request

Welcome to the Software Purchase Request form for A-State Faculty and Staff. This form is designed to facilitate the acquisition of software necessary for your academic and administrative needs. Please fill in contact information, department, and email address. Next, specify the software product you wish to purchase, including the vendors contact information, license details and any additional information required. You also need to provide a description for how the software will be used and documentation, such as a VPAT and  ITS Security Questionnaire or HECVAT from the vendor. Finally, submit the form for approval. Once approved, you will receive a confirmation email with further instructions.
