VPN Access Request for Sponsored Accounts

The Requestor on this form should be the A-State employee who has initiated the creation of the sponsored account. You will provide information regarding the user with the sponsored account on this form. The Requestor WILL NOT BE the sponsored account user.

Because this is access for a sponsored account, this form will route to security for approval. After approval is given, it will be sent to the systems team to grant access. The sponsored account should then receive an email letting them know they have been given VPN access.
